چکیده

Although coronary care units have been successful in reducing the in-hospital mortality after acute myocardial infarction, this is largely a result ofpreventing and treating dysrhythmias. Thefailing myocardium remains a therapeutic challenge and current medical therapy disappointing. Methods ofproviding the heart with mechanical support are described and indications and contraindications to their use are detailed. The results of clinical trials are reported, including the use of bypass pumps and especially the intra-aortic balloon. A recent advance has been the availability of noninvasive support techniques, and results using external synchronous compression are detailed. All methods of circulatory support require careful documentation of claims of their effectiveness. Emphasis should be placed on demonstrating improved myocardial perfusion, preservation of myocardial cells, and reduction in size of the infarct since mortality rates continue to be unacceptably high despite initial haenodynamic improvement even when circulatory support is combined with emergency selective coronary arteriography and direct coronary arterial surgery.

A New Minimum Order Lumped-Parameter Model of Circulatory System for Patients with Suffered Left and Right-Sided Heart Failure

In this study a new minimum lumped electrical model of total circulatory system through numerical solutions to approximate the response of the human circulatory system was presented. Among five existent ventricular elastances as activity function of heart pumps, a more accurate model was chosen as well as the operation of baroreflex system to accommodate variations in heart rate and systemic va...

Decreased expression of tumor necrosis factor-alpha in failing human myocardium after mechanical circulatory support : A potential mechanism for cardiac recovery.

BACKGROUND An increasing number of observations in patients with end-stage heart failure suggest that chronic ventricular unloading by mechanical circulatory support may lead to recovery of cardiac function. Tumor necrosis factor-alpha (TNF-alpha) is a proinflammatory cytokine capable of producing pulmonary edema, dilated cardiomyopathy, and death. TNF-alpha is produced in the myocardium in res...

Mechanical alternans and restitution in failing SHHF rat left ventricles.

We examined mechanical alternans and electromechanical restitution in normal and failing rat hearts. Alternans occurred at 5 Hz in failing versus 9 Hz in control hearts and was reversed by 300 nM isoproterenol, 6 mM extracellular Ca(2+), 300 nM -BAY K 8644, or 50 nM ryanodine. Restitution curves comprised phase I, which was completed before relaxation of the steady-state beat, and phase II, whi...

Regional prolongation of ARI and altered restitution properties cause ventricular arrhythmia in heart failure.

The mechanism of arrhythmogenicity in heart failure remains poorly understood. We examined the relationship between electrical abnormalities and ventricular arrhythmia by using experimental heart failure models. Sixty unipolar electrograms were recorded from the entire cardiac surface in control dogs (n = 13) and pacing-induced heart failure dogs (n = 16). In failing hearts, activation time (AT...

Molecular correlates of altered expression of potassium currents in failing rabbit myocardium.

Action potential (AP) prolongation is a hallmark of failing myocardium. Functional downregulation of K currents is a prominent feature of cells isolated from failing ventricles. The detailed changes in K current expression differ depending on the species, the region of the heart, and the mechanism of induction of heart failure. We used complementary approaches to study K current downregulation ...
